Document submit for apply:

(1) Application form with a photo (passport size) attached

(2) A proof of student registration (請至教務處申請英文版在學證明)

(3) An official academic transcript (請至教務處申請英文版歷年成績單)

(4) Health check result (It must prove that you don’t have any contagious diseases such as tuberculosis or hepatitis)

(5) A copy of the first two pages of the passport

(6) A copy of certificate of medical insurance coverage
