(1) your name in Chinese & English (matching your possport);

(2) Gender

(3) student ID number;

(4) College / faculty

(5) Department

(6) Academic level (undergraduate or graduate)

(7) Year of Study

(8) GPA

(4) mobile phone nubmer;

(5) any proofs of English proficiency test scores
