            (1) your name in Chinese & English (matching your possport);

            (2) Gender

            (3) mobile phone nubmer;

            (4) student ID number;

            (5) College / faculty

            (6) Department

            (7) Academic level (undergraduate or graduate)

            (8) Year of Study

            (9) any proofs of English proficiency test scores

2. Apply to the MBS: by April 30, 2024
